Abstract

Subjective cognitive performance in multiple sclerosis (MS) correlates weakly with objective performance, but is more strongly associated with depression. We aimed to identify symptoms and brain MRI volumes related to subjective and objective cognitive performance. 205 MS subjects, diagnosed within the last 15 years, completed the SymptoMScreen, patient-reported Expanded Disability Status Scale, Neuro-QoL subjective cognitive and fatigue sub-scores, and Beck Depression Inventory. For the objective cognitive assessment the smartphone-based icompanion Symbol test was used. Volumetric variables were calculated from the 52 available brain MRIs using icobrain. A weak correlation was observed between subjective and objective performance (rho = 0.21, p = 0.002). Subjective performance was negatively associated with pain, dizziness, fatigue and depression, while objective performance was negatively related to walking impairment. Objective performance significantly correlated with thalamic volume, while subjective performance did not correlate with any brain volumes. In other words, subjective and objective cognitive performance are related to different clinical markers: subjective performance is linked to invisible symptoms, while objective performance is linked to more visible measurable clinical markers. Integrating both perspectives in clinical practice may provide a more holistic understanding of cognition in MS, allowing for tailored interventions that enhance patient care and quality of life.

Foundational research conducted decades ago established the longstanding speed-centric model of cognitive dysfunction in multiple sclerosis,1-5 which posits slow cognitive speed as the principal deficit that leads to difficulties with memory and other functions.6-8 This model was rooted in an understanding of multiple sclerosis as a prototypical diffuse white matter disease, with cognitive dysfunction likened to subcortical dementia.1
MS creates some real challenges to keep on moving. It helps to inspire each other on how to find ways to move.

Did you stretch in your bed, swing your arms, roll side to side today? Or perhaps you did housework? Or exercised in a pool? Tracked your count of steps? Chased your cat around the home? Pushed the wheelchair wheels with your arms at the store? Post them here and we can support and egg each other on.

REMEMBER: This is a keep on moving thread, not keep on exercising! Any moving is important it doesn't have to be exercise! Even Science proves that.

I've embarked on a revised PT routine, and working the new exercises in takes slightly more time than before. I have 5 new exercises but was able to subtract a couple of others.

The average step count I racked up for April (based on only 24 days) was: 1,016 steps/day, worse than any month since June 2025 but I have excuses:
1 - My back has been a problem ever since one of the PT sessions (over a month ago). It's very gradually getting better but has really slowed me down lately.
2 - The new pedometer is still being put through its paces. On some days it just hasn't recorded anything, and in general it must have a somewhat different sensor than the two pedometers I was wearing before.

Happy May Day!

One of the nicer months of the year in my opinion because the weather tends to be mild and even sunny, with spring flowers popping out everywhere.

What have you been doing lately if you're reading here? If you're reading here, you might as well chime in. The space is here for you to express yourself.

I'm still watching the occasional movie on a DVD from the local library while crocheting hats. I turn out about 30 hats a year, and they eventually go out for distribution among homeless people. There are quite a few homeless people hereabouts, and the winters here are milder than in some parts of the US but they're not really mild enough to survive very well without a roof over your head and some heat.

I'm getting used to the shower chair and liking it. My Talking Books player gave out recently after only about 5 months of use. Usually they last for years. I waited many days just to get a response from the library that sends these materials out but finally heard yesterday that a replacement player is coming my way. This news made my day as I listen to periodicals and books on this device and have been doing so for over 40 years. I'm not blind, not even legally blind, but people with MS are often eligible for the (free) government National Library Service Talking Books program because we have vision problems as well as impaired hand and arm function that makes holding printed material difficult.

That's my story for now. What is yours?
