    Ocrevus improves cognitive functioning

    Ocrevus (ocrelizumab) seems to improve cognitive functioning.

    So, why do you think a person would take ocrevus over rituximab? I'm thinking rituxan is safer, and cheaper. What are your thoughts?

      Because there is concern that insurance won't pay for Rituxan now that Ocrevus is here. What the hell is it insurances business? Rituxan is cheaper.

        I'm pretty sure Rituxan hasn't been approved by the FDA for use in MS. It's been prescribed off label for many people with MS for years but it hasn't been given an OK for MS.

        It's all about marketing. Rituxan and Ocrevus are made by the same company, and they didn't want to seek marketing approval for Rituxan. They've gone with the newer drug, Ocrevus. Without marketing approval the drug wouldn't be affordable because people would have to pay out of pocket for it.

        Somebody posted this before, I think, but this 2015 article makes it clear:

        There's this difference between the two drugs:

        rituximab is chimeric, whereas the ocrelizumab is a fully humanized monoclonal antibody that binds more tightly to its target than the older drug, the investigators said.

        So maybe Ocrevus is thought to be safer and more effective than Rituxan?

          Yes, that's right, it's off label.

          Wasn't it the Wheelchair Kamikaze analysis that brought up the questions?

          It just seems from articles, Rituxan IS the better, safer, cheaper choice, if you can get it.

            I do believe that the answer is found in the money trail. However, it could be that one or the other targets cells that would help one person more than another. I don't really believe this but....

            I wanted to comment on cognitive thinking. I would say that rituxan seems to have helped my thinking and organizational skills. In the last years when John has been so ill I have really taken over business details that require clear thinking. The improvements I have seen did not come all at once or right away. My experience is the improvement has been slow, incremental. Then suddenly I notice that things are much improved. I can do things I had given up on.

            That's all I know.
