Anyone know anything about the Medicare PACE program?

Yesterday the 2018 edition of "Medicare and You" arrived, and leafing through it I found mention of a PACE program but couldn't find out much about it from the booklet.

Today I received a flyer in the mail about events at a local facility I've heard of but didn't know much about--called ElderPlace. I looked it up and found that it is part of this PACE program, and there was a link to the Medicare information about it. My neighbor w Parkinson's Disease was in it. It seemed to be an adult day care for people w chronic disease and included transportation, a meal and medical care.

There are 3 ElderCare options in this city. I don't know if other PACE facilities offer similar arrangements, but here is what the ElderCare assisted living facilities say they have:

ElderPlace-supported housing features:

• Private rooms with private baths (most rooms)
• Kitchenettes (most rooms)
• Private patios with sliding glass door (most rooms)
• In-room emergency call system
• Dining services
• Adult day services and meals
• Recreational and therapeutic activities
• Transportation to off-site medical appointments and ElderPlace events
• Close access to shopping and public transportation
• Pet-friendliness

Residents of ElderPlace-supported housing who are part of the ElderPlace program receive Only registered and activated users can see links., Click Here To Register... not available in other housing facilities:

• On-site primary care from geriatric-trained physicians and nurse practitioners
• On-site dentistry, optometry, audiology and nurse-provided foot care
• Full coverage of all necessary medical treatments, medicines and equipment
• Transportation to and from all medical appointments
• Care coordination by an on-site primary care team

This kind of place sounds like something Sally could have moved to.

The one my neighbor went to was strictly a day time operation. Not a live in place. The Eldercare you mention sounds pretty good.

I read about the PACE/eldercare the neighbor went to. You must give up your docs and use their "team".

Alas, that seems to happen in many nursing homes and apparently assisted living facilities as well.

Surely a doctor who's known you for years would give you better care than someone attached to a facility you're living in? The policy seems like it's a convenience for the facility--and maybe doctors dislike having to visit facilities or have patients with special transportation arrangements turning up at their offices and so they're more than willing to "dump" their patients onto whatever doctors the facility has available?

I wish I could believe that facilities have really top-notch doctors but my guess is that most don't.

There is not one in my community, but is in a town right next to me.

Agate, I do not believe the Doctors would be top notch either. Sad to feel that way, but I do.

I found one PACE facility in Sally's state and a whole flock of them in one of the states next door to her.

Only one in Wisconsin, and not near me. When I was in the nursing home I could not see my primary care doctor, but could see specialist, like my wound care doctor.

As I read the information, though, it sounded like they were trying to keep you out of a nursing home?

My own private PCP was also the medical director at my MILs excellent nursing home. So. I know his patients, and she, got good care there by him. I switched to him as my PCP cuz I was so impressed with the care of my MIL.

So, there may be exceptions?

I was impressed when I walked in and he was sitting by her bed, asking her which ear she heard best with! She always looked forward to his visits, and I know she lived better due to him.

Yes, Cat. Russ, the neighbor w PD, was trying to stay independent for as long as possible. He had a HHA in the morning get him ready and hand him off to the driver. Then Eldercare had him roughly 10A - 4P. There was another aide waiting at the house when he arrived home.

He lived in a three room apartment in his son's house so nights and weekends were covered by family.

Sadly he passed in February. He had terrible lung infections in the winters often requiring hospitalization but he successfully avoided nursing homes.
